The Black Friday Promotion ‘Promotion’ includes the following ‘Program’ at a cost of $497 or payment plan of 2x $297:

  • 1 x 60 minute online Initial Nutrition Assessment Consultation ‘Initial Consult’ via Zoom (valued at $210);

  • 1 x 30 minute online Review and Supplement Advisory Consultation ‘Review Consult’ via Zoom (valued at $105); and

  • 3 month access to PCOS Freedom - QuickStart online learning platform ‘Video Training’ (valued at $500)

There are optional add-ons to the Program Promotion ‘Add-on Promotion’:

  • 3 x 30 minute Review Consultation via Zoom (valued at $315) + 1 month additional access to Video Training (valued at $166.66) ‘3 Review Consult Bundle’ at the cost of $283.50; or

  • 6 x 30 minute Review Consultation via Zoom (valued at $630) + 3 month additional access to Video Training (valued at $500) ‘6 Review Consult Bundle’ at the cost of $504

The Program Promotion is available for purchase for email subscribers from Tuesday 24th November 2020 0800 AEDT & finishes Monday 30th November 2020 1159 AEDT.

The Program Promotion is available for purchase for the general public from Wednesday 25th November 2020 0800 AEDT & finishes Monday 30th November 2020 1159 AEDT.

Initial Consult and Review Consult included in this promotion must be used within 3 months from date of purchase.

Add-on Promotion is available for purchase up to 7 days from date of purchase.

Video Training will be available for access from Friday 27/11/20 0900.

Once a purchase is made via our online store, you will receive an email with a link to access the PCOS Freedom - QuickStart online learning platform and book your Initial Nutrition Assessment Consultation

All prices are in Australian Dollars unless otherwise stated, and exclusive of GST.

By purchasing the Program, you authorise the Company to charge your credit card or debit card as payment for your participation in the Program. Or that you will meet the payment plan as agreed on the program payment page.

All instalment payments will be deducted automatically from the card held on file on the invoice date. Automatic reminders will be generated by the payment system on the occasion that the payment has failed. If payment fails on a total of 3 occasions over a 5 day period, you will be excluded from access to all program elements including Initial Consult, Review Consult and Video Training.

Availability

Initial Consult and Review Consult can be made on Tuesdays & Thursdays between 0900 – 1800 and Fridays between 0900 - 1300 between the 1st December 2020 0900 to 2nd February 2020, subject to availability. Appointments outside these days/times may be available but not guaranteed.

Consultation days & hours are current as per the 24th November 2020 however can be altered without notice.

Cancellation Policy

You may cancel or reschedule your Initial Consult and Review Consult without charge at anytime 24 hours before your appointment.

Failure to cancel within 24 hours will incur a forfeit of the Initial Consult or Review Consult. Cancellation policy can be waived for Medical emergencies or exceptional circumstances deemed appropriate at The Diving Dietitan’s discretion.

Refunds

Program Promotion comes with a 5 day cooling off period from date of purchase, (5 day cooling off period includes weekends and public holidays) and is subject to an administration fee of $50. No refunds will be given after 5 days from the date of purchase unless necessary under Australian Consumer Law.

By purchasing the program, you agree:

(1) All materials and information provided to you by the Company are confidential and proprietary intellectual property belonging solely and exclusively to the Company, and may only be used by you as authorised by the Company; and

(2) The reproduction, distribution and sale of these materials by anyone but the Company is strictly prohibited;

(3) You have sought medical advice from a doctor and have undertaken medical investigations to be given a diagnosis of Polycystic Ovarian Syndrome (PCOS);

(4) The advice and treatment recommendations provided by the Company does not replace medical advice. We encourage you to seek medical advice promptly from your doctor if in doubt at any time during the program;

(5) By engaging the services of The Diving Dietitian, you acknowledge that you are solely responsible for your results and you take full responsibility for your health, successes and failures as a result of your own effort;

(6) Individual results are not guaranteed and may vary. Prior results also do not guarantee a similar outcome;

(7) You accept the risk that results differ for each individual and The Diving Dietitian will not be liable if you do not achieve the results you desire. Results may differ depending on each individual’s background, dedication, desire and motivation. As with any health-related programs or service, your results may vary, and will be based on a whole host of variables, including but not limited to, your individual capacity, life experience, unique health and genetic profile, starting point, expertise, level of commitment and follow-through. You are accepting full responsibility for your own actions and results, and The Diving Dietitian will not be liable or held responsible for losses or damages as a result of the use, or non-use of information or advice provided;

(8) The Participant will not hold The Diving Dietitian responsible for unforeseen technical difficulties including to but not limited to internet outages for both parties. We will make every effort to fix the problem on our end in a timely manner.

We may occasionally recommend supplements or nutrition-related products as part of your treatment plan during your consultation. Our dietitian makes these recommendations based on the information you have provided about your health, medication use, allergies and intolerances. We do so to the best of our knowledge on drug-nutrient interactions and dietetic training.

Dietary products are not intended to treat, prevent or cure diseases. Do not stop taking medications without speaking to your doctor. We are not liable for negative outcomes that may arise if you have not disclosed relevant information to us which may affect your treatment plan and supplement recommendations. Individuals are different and may react differently to different products. You should always consult your doctor and read information provided by the product manufacturer or product label before commencing any new supplements, nutritional products or dietary program. Always check the product label or packaging for product information, directions for use and storage instructions prior to using any products. If in doubt, you should contact your dietitian immediately or contact the manufacturer directly for clarification.

This contract originates and is executed in Victoria, Australia and all disputes will be administered under relevant state legislation regardless of the location of the Participant.

By purchasing online, the Participant indicates complete understanding and agreement to the Terms and Conditions outlined in this contract.
